- Factual errors: In the scene where Wendy calls Kevin at the cemetery, Wendy says she is at Jason's grave. When she is seen kneeling by the grave, the grass below her shows no sign of any kind of recent burial like the movie portrayed.
- Continuity: When the tanning bed owner puts the tube of lotion under the door, the position of the door changes depending on the shot.
- Continuity: The direction that Ian holds his nail gun changes between shots.
- Continuity: The position of Ashley's earphones changes as she panics when they are trapped in the tanning bed.
- Continuity: During the final scene when Kevin is thrown against the window of the subway, the direction of his head changes between shots.
- Revealing mistakes: At the very end right before the camera shows the main character you see a dead body in the lower right portion of the screen and right before the camera cuts away you can clearly see them blink.
- Continuity: In the scene in the tanning beds. When it cuts to Wendy and Julie. Right before Wendy says "Julie, I need a minute... in private." it shows a back shot of her. She's holding the crumpled paper that has Ashley's cell phone number on it. After Julie leaves, Wendy looks to her left and the paper is out of her hands, on the desk next to her.
- Continuity: When Wendy first looks at the photo of the passengers on the roller-coaster, there is no evidence of the person in front being Julie, only her hat is visible. However, when Wendy looks again at the photo later on, Julie's hand is on her head and the good luck bracelet is visible.
- Continuity: In the tanning bed scene just before the girls catch on fire, Ashlyn pushes her chest up and gives one big scream. However, in the reflection of the facial lamps Ashlyn is doing something completely different - she is shaking her hand frantically right next to her face.
- Incorrectly regarded as goofs: When the rollercoaster is climbing the hill in the premonition people say that Frank disappears from the rollercoaster until you see him filming Ashlyn and Ashley on the lift. But if you watch it properly, Frank is still there but you only see him for a brief moment because there is a very dark shadow is covering him.
- Audio/visual unsynchronized: When Wendy and Kevin come out of the Police station, after the first angle change, their spoken words do not match their lip movements.
- Continuity: When Ashley and Ashlyn first enter the tanning bed room, a stereo and the control panels for the tanning beds can be seen on the opposite wall. During the over-head shot, (when the beds are on fire) the control panels can be seen, but the stereo is gone.
- Continuity: In the gym, the exercise machine that Lewis dies on has a flat surface when he first sits down in it. Later, in the shot of the weights coming down, the seat is big and bulky.
- Factual errors: The "last photo of Lincoln" is shown as the character points out that the crack in the picture (on Lincoln's forehead) as "exactly where he was shot". Booth shot Lincoln from the rear, below the left ear with a small Derringer pistol. There was no exit wound.
- Errors in geography: The film is set in Pennsylvania, however the 'Build It' store prominently displays signage for Rona Home & Garden, a Canadian home improvement store.
- Crew or equipment visible: After the Lewis scene in the gym, a camera can be seen reflected when Kevin opens the door after Wendy asks him to drive.
- Continuity: In the tricentennial scene, a black guy in purple can be seen jumping out of the way of the horse. In the next shot, that was just a split second later, he can be seen ordering popcorn.
- Continuity: Wendy's hair in the cemetery scene changes between shots
- Continuity: The people behind Kevin when Wendy talks to him out in the rain (after getting her diploma) in a shot are clearly Julie and Perry. But in the next shot, there are different people.
- Continuity: On the DVD feature "Choose Their Fate", whenever you choose for Wendy to take another look at Louis's picture, in the regular version the swords cut the cable that holds the weights. If you choose she does take another look, the weights just squash his head without the swords cutting the cable to it.
- Factual errors: It is highly unlikely that two tanning beds would be in the same room together. They would be one bed per room because you have to go naked or in a bikini.
- Factual errors: In the tanning bed room, when the temperature controller's display changes from 77 to 78¡F, it can be seen that the fan control switch on the lower left side is set to "On". This should normally have the fan (the one built into the wall, not in the tanning beds) running all the time. However, it is then shown how it turns on due to increased temperature in the room and makes the coat hanger wobble. The switch should be set to "Auto" if this were to happen.
- Audio/visual unsynchronized: In the Choose Their Fate, on Erin's alternate death, as the birds fly in her face and she falls back, you can still hear the sawdust bag being popped, even though the spike never hit it.
- Revealing mistakes: When the speeding car rams Kevin's car into Frankie's car, due to the laws of physics, Frankie's car should have gone forward but instead stays in the same position.
- Factual errors: Roller coasters have mechanical fail-safes that keep the restraints down even if hydraulic pressure is lost.
- Revealing mistakes: Once the train has crashed and Wendy is on the tracks you can see her look back to all those who died. In the left middle of the screen near the top of the train piece you can clearly see a man breathing as well as blink and move his lips as if he were talking.
- Revealing mistakes: When Wendy is at her house looking at the pictures on her computer, she sees Jason's picture.He's striking the same pose as the devil and we see the roller coaster going down a hill and it looks as if it was going to hit him in the head. However, when Julie takes the picture at the beginning of the movie, the roller coaster had already passed by the devil and nowhere near Jason.
- Revealing mistakes: At the beginning of the movie at the fair, Wendy is standing next to the roller coaster tracks. As the roller coaster passes her on her right side, a gust of wind supposedly from the coaster blows her hair. The funny thing is though, the wind came from the left side instead of the right where the coaster passed her.
- Factual errors: In the tanning salon sequence the tanning bed is locked down by the rack of CDs above it that fall. Tanning salons are required to put any hanging objects elsewhere. Many salons have warnings in the room not to hang or set anything above the tanning bed...for obvious reasons.
- Factual errors: A cordless nail gun takes about 2 seconds to recharge, and cannot fire consecutive nails nearly as fast as depicted.
- Factual errors: Fireworks mortar tubes must be at least Schedule 40 ABS tube, not the lightweight Schedule 80 PVC tube shown. The lifting charge would immediately explode through such a thin tube. Fireworks are never launched without maintaining a visual reference to the mortar racks (which incidentally are banned by law to be placed on rolling trailers), allowing the pyrotechnician to immediately cease launching if there was any damage or instability to those racks.
- Factual errors: In Lewis' alternate death, the swords never cut through the wire. You see Lewis doing several reps when they walk in so the wires, at that point, are still in place. The swords don't even move, then Lewis does one final rep and his head is suddenly crushed by weights. Nothing could have cut the wire between then.
- Errors made by characters (possibly deliberate errors by the filmmakers): When Kevin tells Wendy about the plane crash six years before, he says seven people got off the plane, same as the roller coaster. But ten people got off the roller coaster.
- Continuity: Kevin's photo shows a shocked expression as a camera flash has gone off close to his face. However, in the scene where Wendy takes this photo, he throws his head back, making this expression after the flash has gone off, therefore after the picture was taken, resulting in a completely different picture.
- Factual errors: When exiting the police station the sign on the police station doors reads "State of Pennsylvania." In reality the state's official name is the "Commonwealth of Pennsylvania".
- Continuity: At the tricentennial event, Kevin burns his face in a fire. You can see that his face is scarred when Wendy and Julie are helping him walk away. However, at the end, when they are on the train, Kevin's face is completely clear.
- Continuity: When Wendy says she will recharge the camera for Julie, she plugs it into the CPU of the computer and looks at the pictures. Later, when she uploads the pictures, she plugs the cable into the camera and the other end into the CPU of the computer. She had already done this earlier, though.
- Factual errors: Rollercoaster Seat Restraints do not work with hydraulic fluid. They work with pressurized air to enable fast release in case of an emergency. The moment the braces shut, the computer checks for irregularities in the locking system prior to clearance.
- Continuity: In the premonition, Frankie drops his video camera and it wraps around the track and causes havoc. When everyone gets off the ride he has his camera with him, but the coaster still crashes even though the camera was not there to wrap around the track.
- Plot holes: In the roller coaster premonition, Frankie's camcorder falls out of his hands and is instrumental in the roller coaster cars' wheels coming dislodged, leading to the majority of the accidents that occur. After the premonition is over, Frankie gets off the coaster, meaning that the accident may not have happened. Even though the hydraulic pipe was leaking, it was only after the car drove over Frankie's camcorder that the major damage occurred.
- Factual errors: In the tanning bed scene, the girls have no way to turn the bed off, but in real life all tanning beds have a START and STOP button right in the bed.
- Continuity: Wendy uses the schools camera for the yearbook, saying that she has to turn in the pictures for a deadline. But in the scene where she is picking up her diploma everyone already has their yearbooks. Later Julie asks to use the camera to get pictures of graduation to meet deadline.
- Boom mic visible: In several shots during the weight room scene, a boom mic is reflected in the hanging swords.
- Crew or equipment visible: When both Kevin And Wendy are looking at the pictures after the funeral, Kevin asks Wendy to look at Frankie's picture. Kevin then turns the car a little to the left almost hitting the tow truck, seen in the metal bumper you can see the front of the camera truck.
- Revealing mistakes: The rollercoaster used for the Devil's Flight is really the Corkscrew at Playland in Vancouver, where all three movies were filmed. If you look during the opening credits, you can see the name "Corkscrew" up in the lights above the coaster. After this, it says Devil's Flight during the rest of the rollercoaster scenes.
- Incorrectly regarded as goofs: The cause of the roller coaster crash was a shaky wheel hitting Frankie's camera after he drops it during the loop. However, Frankie is one of the people who get off the roller coaster after Wendy's outbreak thus removing a factor that caused the crash. However, it was established earlier that before Wendy's vision started, that the train's fluid lines were already faulty. Also, the break in the track that caused Wendy's car to flip at the end of her vision was already there. The train would have crashed anyway; Frankie's camera only made the existing problems worse.
- Continuity: In Wendy's vision of the roller coaster crash the break line snaps because Franky's camera was wrapped around the tracks. Franky got off the roller coaster so his camera couldn't have wrapped around the tracks, and the break line would not have snapped.
- Continuity: When Wendy sees the crash of the roller coaster it show that Lewis doesn't fully put down his over head bar. The workers notice and slammed it down the the camera shows that it had started to leak helping the cause of the crash. When Wendy starts freaking out and gets off the coaster, Lewis is also one of the people that got off of the roller coaster.
- Revealing mistakes: In the beginning when the girl is taking photos. The digital camera makes a 'reloading' noise which is only found on film cameras.

Goofs below here contain information that may give away important plot points. You may not want to read any further if you've not already seen this title.
- Continuity: SPOILER: In the "Choose Their Fate" alternate version of the tanning-salon scene, Ashlyn reaches out with her left hand to pull Ashley out of the tanning-bed, just before they're both electrocuted. In the close-up shot of Ashlyn's body hitting the floor, her left hand is on the floor and empty, but in the overhead shot immediately afterwards, her left hand is still clutching Ashley's.
- Continuity: SPOILER: In the "Choose Their Fate" alternate version of the tanning bed scene when the girl standing grabs the others hand and is electrocuted her face and body are burnt but when it shows her drop to the floor dead, her skin is perfectly clear.
- Revealing mistakes: SPOILER: When Kevin's car is pushed into Frankie's car by the runaway truck, you can see two shots of the engine flying out: an overtop and a sideview. In the overtop shot Frankie's car is empty, in the sideview one, he can be clearly seen. Assuming they are the same shots, this shouldn't happen. Also, assuming he is covered by the sunblocks, one can easily notice they are semi-transparent, so at least his hat should be visible, if not his head.
- Continuity: SPOILER: When Kevin's truck rear-ends Frankie's car, the overhead shot shows the engine departing the truck and flying forward over the back half of Frankie's car. In the next shot, this time from the right hand side, you see the engine once again shoot out of the truck, and fly forward over Frankie's car.
- Continuity: SPOILER: When the engine cuts Frankie's head, it can be seen that his eyes end up closed, however when Kevin and Wendy check who is in the car, Frankie's eyes are open.
